Choosing the Correct Training Form

Please read the form name carefully before downloading it. Supplying the correct information helps FACT Adelaide process your request efficiently.

• Refund and rescheduling: Use this form when requesting an eligible course change, cancellation or refund.
• Corporate or group bookings: Use this document when arranging training for employees, contractors or workplace groups.
• Student Handbook: Review important information concerning enrolment, training, assessment, student support and your responsibilities.
• Third-party authority: Complete this when authorising another person or organisation to communicate on your behalf.
• Under-18 consent: A parent or guardian must complete the relevant form for students younger than 18.
• Complaints: Use the complaints form to provide details of a concern and the outcome you are seeking.
• White Card replacement: Use this form when requesting a replacement for an eligible White Card previously issued through FACT Adelaide.
• Interpreter assistance: Submit this form if language assistance may be required during your training.

Before Submitting Your Forms

Check that every required section has been completed and that your contact information is correct. Include supporting documents where requested. Incomplete forms may require follow-up before they can be processed.

Students undertaking nationally recognised training may also require a Unique Student Identifier. You can create or manage your USI through the Australian Government USI service. General information for vocational education students is also available from the Australian Skills Quality Authority.
